Important: Therapy notes
Dried prothrombin complex (Beriplex®) is used in addition to vitamin K in patients who are bleeding and on warfarin or in whom surgical intervention is planned imminently and the bleeding risk is thought to be unacceptable (note for certain procedures reversal may not be required; discuss with Haematologist). It provides temporary but immediate reversal of anticoagulation. It can be obtained from the Blood Transfusion Service on the authorisation of a Haematologist.
Note:
- Idarucizumab (Praxbind®) is used for the reversal of dabigatran, prior to emergency surgery or in patients with life-threatening or uncontrolled bleeding.
- Andexanet alfa (Ondexxya®) is used for the rapid reversal of apixaban or rivaroxaban, in adults ≥18 years who have taken either drug in the past 48 hours and present with life-threatening or uncontrolled bleeding.
- There is no licensed antidote available yet for edoxaban. Discuss with haematologist regarding reversal options for edoxaban.
