Empower people to self-manage their condition

There is substantial evidence to support the value of personalised actions plans for asthma in both adults and children and this is a benchmark for quality asthma care. Resources to support holistic person-centred care and signposting to wider resources
- Do practitioners and local co-ordinators know and have lists available of local groups, e.g. walking groups, weight management groups, sports centres?
