This resource has been developed to support staff working as part of a multi-disciplinary team within a Mental Health and Wellbeing in Primary Care Service. It is intended to help all staff, by providing a directory of additional sources of help and support, that people can be connected in with and signposted to, to compliment the support, assessment and treatment they will receive from the team.
