Conversation pre-referral
Referral made and assessed
Referral accepted and person notified in real time by text SMS or email that they have been accepted and placed on a list
People could self-select e.g. go onto NHS Inform, or read in a leaflet, see a poster - thinking about their own self-assessment - and if they want a wellbeing discussion to plan a preventative and proactive waiting period they can opt in
Universally triage for some key groups - at the start of wait - send a text, it takes them to a DrDoctor questionnaire - and for people that seem to have lots they want to look at, we contact and see what support can be offered
Risk stratify people waiting that we believe to be at higher risk of waiting being harder e.g. age/ Scottish Index Multiple Deprivation/ disability etc.
Preventative and proactive wellbeing discussion (face to face/ Near Me/ phone) and talk through what matters to the person, what their risks and resiliencies are, what they want in their plan - what they can self-care and focus on etc.
People start their plan + people have follow up support
