If you are newly diagnosed with dementia you are entitled to receive a minimum of one year post-diagnostic support (PDS), from a specially trained Post-Diagnostic Support Link Worker. Within Inverclyde Health and Social Care Partnership it is usually the Older Adult Mental Health Team who offer memory assessment and give a dementia diagnosis.  If you are diagnosed with dementia by this team you will routinely be offered Post Diagnostic Support and a referral will be made to the PDS Link Workers, who will contact you to start offering support. 

The post-diagnostic support is designed to help you:  

  • Help you understand your illness and how to manage your symptoms 
  • Keep you connected to your local community and make new connections 
  • Have the opportunity to meet other people with dementia and their families 
  • Help you plan for your future 
  • Help you plan for future support
