HPI Vulnerable


Mum has limited understanding of Jack’s language, father and other familiar adults struggle to
understand what he is saying. Jack becomes frustrated at this.


Single words (estimate of 40-50) Sounded vowel errors, initial consonants missed, uses ‘h’ for a
variety of sounds.


Attempting to join words together 2 word/ occasional 3 word sentences e.g Mummy cookie, Ice
cream please.


Mum concerned and requesting support.


What are the family, you or your agency currently doing to support this
child?


Speech bundle provided- HV discussed SPEECH tool highlighting specifically the
Play & interaction, Expressive and Speech sounds.


Encouraged parents attending the SALT drop in.


Nursery application encouraged & peer play through pre 5 groups.


Mum has agreed to work through SPEECH and be review in 12 weeks by HV.


Are you aware of actions from any other agency, being taken to support
child/family currently, or in the past?


No.


What do you consider another professional can do to help the child's
wellbeing?


Assess Jack’s' speech and language development and guide parents on any
advice/strategies/intervention as required.


What difference to the child's well-being is the practitioner (requesting
assistance) hoping to achieve?


To reduce Jack’s' communication difficulties.

 

To increase Jack’s' vocabulary.


Jack’s speech sounds to improve for others to understand.


To socially communicate with peers through play to role model and promote a learning environment
for age/stage developmental progress.
