NHS Highland Patient Agreement for Investigation or Treatment Consent Form
This is the generic consent form across NHS Highland. (NHS Highland intranet access required).

NHS Highland has recently amended its consent form to enable an informed consent process. It is a tool to prompt and record the consent process and to ensure that the patient has as much information as possible regarding their procedure including: risk; benefits; alternatives; etc. More information on informed consent can be found on NHS Inform.

Please note that some specialties use consent forms specific to their specialty and not the generic consent form.

As part of informed consent it is our aim to provide patients with as much detail as possible with regards to their procedure. We encourage all patients to browse our patient information section as well as NHS Inform.
