Base the choice of emollient on patient preference to optimise compliance, preparation cost, site of application and whether it is for use only as an emollient or also as a soap substitute.

MHRA advice: Emollients and risk of severe and fatal burns: new resources available (August 2020) (www.gov.uk).

Warnings about the risk of severe and fatal combustible burns are being extended to all paraffin-based emollients regardless of paraffin concentration. Data suggest there is also a risk for paraffin-free emollients. Advise patients who use these products not to smoke or go near naked flames, and warn about the easy ignition of clothing, bedding, dressings, and other fabric that have dried residue of an emollient product on them.

Aqueous cream has been removed from the Highland Formulary as an emollient because it commonly irritates the skin. It was not designed to be left on the skin and should only be used as a soap substitute if at all. Newer, less irritant emollients such as Zerobase® cream and Zeroderm® ointment should be recommended to patients; for further information refer to guidance on the Use of emollients.
