Daily care

Use pressure relieving devices such as mattresses and cushions to minimise skin breakdown. 

Depending on the condition of the client their skin may be very thin, dry and damaged in areas. Local policy should be adhered to when dealing with any broken or damaged skin. 

Any new breaks in the skin should be reported to the community nursing team. 

Assessment tools should be accessed and used as per local policy. 

 

Moving clients

Local policy should be adhered to when moving a client who has mobility issues.

Frequent repositioning of the client is important to minimise skin breakdown.
