Action Card: General Anaesthetic/Critical Care Staff

Objectives

Overview:

  • During working hours, ICU consultant will become Critical Care Consultant 1 (CCC1)
  • Outside of working hours, On Call consultant will become Critical Care Consultant 1 (CCC1)
  • Consultant Critical Care (CCC) Leads will be assigned to the following clinical areas: Resus; Theatre Corridor; Green ICU and Red ICU.

  • All other anaesthetic/critical care staff should be allocated to support clinical activity in these areas as necessary.
  • CCC leads will coordinate activity in each area.
  • Theatre Control Team will be based in the Anaesthetic Seminar Room
  • 36 SJH consultants on Major Incident (MI) Whats app call out (44 Total including staff grades)
    • First 10 available should report immediately for allocation. Go to Anaesthetic Seminar Room, Theatre Corridor.
    • Further 10 should await more information, rest and plan to be available to cover the next shift/attend as necessary
    • Anticipate that 10 are on leave/unavailable
    • Consider wellbeing champion of numbers allow
  • First 8 trainees should attend, further trainees should await further information, rest and plan to be available. Where possible a mixture of trainees grades would be helpful, ensuring that there are at least 1-2 obstetric competent trainees available.

 

General Duties:

  • Respond to any major incident (MI) call out
  • Attend SJH theatres are requested
  • Respond to requests for assistance from Critical Care Consultant leads:
  • Attend to patients in ED, Theatres, Critical Care, Obstetrics as requested
  • Maintain, wherever possible, normal routes of communication and referral
  • Be prepared to update area leads about current activity
  • Look after and support your colleagues

 

Initial Actions:

  1. Respond to a major incident call out-
  2. If weekday day time then please expect to attend if you are currently in theatres or in office
  3. Out-of-hours ensure that you reply to call-out with name, availability and estimated time of arrival in theatre suite. Proceed to hospital.
  4. Non-trainee grade: If due to assume on-call/bleep holder role in next 12 hours, please do not attend until that time
  5. Trainee: If due to assume on call/bleep holder role in next 24 hours, please do not attend out with usual roster hours
  6. If currently in theatre with an ongoing case then aim to complete case as rapidly as can be safely achieved
  7. Patients currently in theatre should be transferred to a recovery area designated by the theatre coordinator (not main recovery)
  8. Report for incident by signing in on sheet in main recovery and proceed to day surgery coffee room to await further instruction
  9. Ensure that you have your ID badge with you at all times

 

Continuous Actions:

  1. Follow task requests from Critical Care Consultant 1 or their designated clincal area lead
  2. If allocated to a Critical Care Consultant (CCC) lead role:

            - Ensure your bleep/phone number is distributed via Whatapp MI groups and recorded by Theatre Control Team

            - Attend your clinical area, coordinate you team and report regularly to CCC1 in theatre control team.

  1. Communicate likely requirements of patients coming to theatres or ICU to the Theatre Control Team
  2. Ensure all patient documentation is complete
  3. On completion of task report back to CCC area lead or CCC1 and proceed to coffee room if not immediately required
  4. Do not leave hospital without reporting to CCC1 and signing out
  5. Be prepared to be sent away to return at a later time if staffing requires
  6. Refer all external requests for information about incident to the control room
