1. Alcohol and Drugs, problematic use in pregnancy flow charts

All pregnant women using;

  • illicit opioids and benzodiazepines
  • Prescribed opiate substitutes (O.S.T)
  • Prescribe opioids above recommended dose
  • Prescribed benzodiazepines above 20mg daily or equivalent
  • alcohol above recommended limits

Should be referred to SNIPs/safeguarding team with a view to transferring care. If women decline transfer, liaise with SNIPs re management.

2. Management of substance use in pregnancy

Inpatient management of women on opiate substitute medication.

3. Inpatient management of women on prescribed opioid substitute therapy (OST)

Missed Doses

  1. Applies to inpatient care when medication has been daily dispensed.
    (for medication which is not daily dispensed, see guideline 2).
  2. Applies to missed doses over 48hours age, and less than 7days.
    (for doses missed within last 48hours, prescribe as normal. For doses missed for 7 days or
    more, restabilize as per guideline 4).
