Adult patients may be referred by another health professional, but may also wish to self-refer.
When you come to the Cleft Team as an adult (15+) wanting further input, your first appointment is used to discuss any concerns you have about your cleft, and/or any input you would like at this time. From this discussion, you will be referred to the appropriate member(s) of the Cleft Team.
All new or returning adults must complete the below referral form and questionnaires (Adult Assessment Pack) and return them by post to the address as follows:

The first appointment is held by a Clinical Psychologist from the Cleft Team - this appointment is not a therapy appointment, but a discussion about the input you would like at this time. However, as part of this you will be asked about feelings about your cleft and general wellbeing, to help plan your care.
