Each individual risk should be specified in the care plan with indicators of an increase in risk identified. Current level of risk of each specific situation occurring should be estimated as far as possible. A clear risk management plan should be in place for each identified risk, with roles and responsibilities made explicit. Consideration should be given to the use of a recognised tool such as STORM in assessing the particular risk of suicide. The overall aim is to minimise the risk of harm to the patient and those around them, while always promoting the personal responsibility so vital for recovery.
