Neonatal community outreach team (NCOT)
Criteria for NCOT referral:
- Baby’s CGA above 34 weeks gestation
- Medically stable
- Temperature stability in open cot
- No significant weight loss and adequately feeding – nasogastric / breastfeeding / bottle feeding and/or combined feeding
- Weight above 1.6kg
- Parents have been educated with necessary skills
Types of referral considered:
In-patient referrals:
- Complex babies who need continuity of care e.g. congenital anomalies with palliative care needs
- Baby who needs nasogastric tube (NGT) feeding support until full oral feeding is established
- Oxygen dependent babies
- Babies with neonatal abstinence syndrome (NAS) who are on a stable reducing regimen
- Babies whose families need reassurance and short term support (please discuss with community band 7)
- Babies who require feeding, nutrition and growth monitoring
- Babies who require home phototherapy
Community referrals:
- Early jaundice
- Large weight loss (more than 13% from birth weight)
