add IV gentamicin if severe sepsis

Consider adding IV amoxicillin for Listeria cover up to 6 weeks of age, if meningitis cannot be excluded.

1 month and above: Hospital-acquired

First line

IV piperacillin/tazobactam
add IV gentamicin if severe sepsis
