Dosing schedule should be reviewed/adjusted in reduced renal function - refer to BNFc and/or liaise with Pharmacist.
Piperacillin/tazobactam can be administered by intravenous infusion or bolus (i.e. injection over 5 minutes, off label), as per Medusa.
