How to complete a Notification of Concern (NOC)
1. All NOC/child protection concerns should first be discussed with the CIC/senior decision maker
2. Select your patient on Trackcare and go into Clinical Portal
3. Select ‘Forms and Pathways’
4. Select ‘Add new form’
5. Select ‘Notification of concern (child protection)’
6. In Part 1A:
Enter your own details as the referrer
7. In part 1B the designated person is Dr Colin Bell-his email address is Colin.Bell@ggc.scot.nhs.uk
8. In part 2B enter the details of the social worker you have spoken to
9. Fill in the remainder of the form in as much detail as possible. The most important area is section 7. Social work do not have access to clinical notes on portal. It is essential you describe the clinical details/your concerns, any discussion with social work and the outcome (i.e., patient admitted or discharged)
10. All intoxicated adolescents require follow up by the youth health service. In section 7 document “refer YHS”
11. Click ‘Complete’ to finish and save the form
12. Click ‘Forms and Pathways’ then ‘Completed forms’ and check form has saved correctly
13. Put a patient ID sticker in the Child Protection book. Nadine Robertson (ED secretary) will email copies to social work and the Child Protection team on the next working day.
