About this checklist

This nutrition checklist has to be used by Care at Home Services within NHSGGC and should not be used in place of MUST in other services within NHSGGC

Malnutrition continues to be a pressing issue and crisis, which is still under-recognised and under-screened. The Patients Association Malnutrition Checklist was developed in 2018 to address a gap that existed for self-screening tools that were simple and easy to use. The checklist helps patients and staff working in health and social care to identify the potential risk of undernutrition in adults, aiming to encourage conversations and raise awareness of the potential for undernutrition.

Content management and governance

The content of this toolkit has been developed by The Patient Association and adapted for use within NHSGGC by Claire Stewart, Practice Development Nurse for Food, Fluid and Nutrition and Joanne Logan, Practice Development Dietitian for Food, Fluid and Nutrition.

The Content has been approved by the NHSGGC Community Food, Fluid and Nutrition Partnership Operational Group.

The Content will be reviewed annually.  User feedback will be reviewed regularly and changes made if appropriate. 

 

 

 

User testing

User testing carried out by care at home services within East Dunbartonshire and Renfrewshire HSCP's.

Accessibility checks

Content has been checked for accessibility in line with the relevant RDS standard operating procedure.

The content was approved by NHSGGC Community Food, Fluid and Nutrition Partnership Operational Group 

Monitoring and acting on feedback and complaints

A feedback from has been added to the toolkit. 

This feedback will be regularly reviewed and acted upon.
