Welcome to the NHS Borders RefHelp Toolkit, a resource to provide information to primary care about referral pathways into secondary care and other community services. This resource is under development to replace the old refhelp site which is not currently actively maintained.

Colleagues will note where specialty referral guidance is available a corresponding icon appears above that specialty. Where referral guidance is in development, a construction icon is visible.

As this is a new resource which is still being developed, we would welcome any feedback regarding the content or wider resource and structure.

What to expect upon referral: Active Clinical Referral Triage

The following guidance 'ACRT and PIR toolkit' is accessible via TURAS. The below diagram is taken from this resource:

Patients should be advised that referral from primary into secondary care will have one of 4 potential outcomes, namely:

  • a virtual appointment offered via nearme or phone call
  • a face to face appointment (which should only occur if there is a clinical need)
  • diagnostics
  • clinical information supplied and opt-in process applied*

*The main aim of the Opt-In process is to improve an individual’s knowledge and facilitate shared decision making. Clinical information regarding the complaint and possible options, including self-care, is provided immediately after ACRT using appropriate booklets or websites. The patient is then empowered to decide if and when to contact the service regarding their problem, with no time limit. In some cases, this information may be sufficient, but where an interaction is still sought prior provision of information is important to facilitate an informed discussion of the available management options which is also crucial for the consent process.
