Being a partner in my care provides you with tools and resources to take more control over decisions about your care, based on what matters to you. It will help everyone to discuss their  needs and views with healthcare professionals. This will all help to put your needs are at the centre of your care. 

Being a partner in my care supports a new approach to healthcare called 'Realistic Medicine'. Realistic Medicine is changing the way healthcare professionals work.  It is an approach to healthcare that aims to put people, including patients and carers, at the centre of decisions about their care and wellbeing.
