Non-severe CAP (CURB65 score ≤2 without features of sepsis)

First Line

Important: Therapy

Amoxicillin 1g PO 8 hourly

 

If atypical pathogen suspected (e.g. upper lobe changes, travel/exposure history) ADD:

Doxycyline 200mg PO STAT, then 100mg daily

 

 

Notes:

If oral route is compromised:

Amoxicillin 1g IV 8 hourly (+ Clarithromycin 500mg IV 12 hourly if atypical pathogen suspected)

Send sputum for culture if recent antibiotic exposure (e.g. in primary care prior to admission)

Review travel history and animal (especially birds) exposure and consider investigation for atypical pathogens.
