Step 7: Accessing and using the Key Information Summary (KIS)

When the GP practice returns a copy of the KIS, file it with the DNACPR (where appropriate) at the front of your resident’s care plan.

Make sure everyone knows where the KIS is kept and when to access it. Share the KIS with professionals (eg, NHS 24, Out of Hours, Paramedics, GPs, District Nurses, Care Home Support Teams, Therapists, Hospital Teams) when decisions about the resident’s care and treatment are being made.

The care home acute clinical incident flowchart provides guidance on how to use the ACP-KIS.
