Step 4: Discuss and complete ACP in care homes – talking & making a plan

The following guidance provides a conversation model to follow when having your ACP conversations (REDMAP), and a set of ACP questions to enable you to make a plan with residents, their families and friends.

Work through each stage of REDMAP before asking the ACP questions. Each stage is important and should be followed in order. This will ensure everyone is prepared to make a plan which is appropriate and reflects the circumstance and wishes of the resident. Do not ask the ACP questions without preparing yourself, the resident and their family first. For this reason, the form with the ACP questions should not be posted to families to complete.

There are two versions of this guidance:

  • a version for your discussions with residents and families or friends
  • a version for your discussions with families and friends of residents who do not have capacity.

Use the most appropriate version to guide your discussion.
