We know it can be difficult to break contact with a team you have known well for a long time. After you have transferred to an adult team and the transition process is complete it is important for your continuing health that any questions or concerns are discussed with the adult team. The adult team and your GP should be your new point of contact for all matters relating to your health.
Transition to adult care
Transition is the process of changing from one condition or state to another.
In healthcare, we use the term 'transition to adult services' to describe the process of preparing, planning and moving from children's services to adults.
This can be an unsettling time, when you are moving from a team of health professionals that you know well, to a new service which is unfamiliar. We will actively involve you in the transition process, giving you confidence in the move.
Transition will be a gradual process and you and your family will be supported through it by your healthcare team. They will discuss the process with you and let you know what to expect. They will be happy to answer any questions you have and direct you to any other information you need. If you want more information on transition of care, please ask any member of your healthcare team at your next appointment. It can sometimes help to write down any questions you have and bring these with you.
If you see healthcare teams in multiple specialities you may not need to transition to adult healthcare providers in all of these areas, as your healthcare needs can change as you get older. Some specialities will give you contact details of the adult services available to you if you develop problems in the future. If you are unsure about this, or what to do in the future, just ask.
Many teams in the hospital use the Ready, Steady, Go programme. This is also widely used in other hospitals. Find out more about Ready, Steady Go.
