What is hospital at home?

The Hospital at Home service provides hospital level care to you at home. Your condition would otherwise need acute hospital admission. We also review those patients at high risk of admission because of falls, providing the gold standard comprehensive Geriatric assessment.

The service operates from 8:00am to 6.00pm, Monday to Sunday

Why have I been chosen for Hospital at Home?

You meet the four A's criteria, which are:

Age - majority older adults, but anyone over the age of 18

Area – Eildon, Cheviot, or another locality, subject to service capacity.

Appropriate care - independent or care needs met by care givers or family

Attendance - needs met by Hospital at Home team based on clinical need, either by telephone consultation, or by visits up to 2 times a day.

Who are the team looking after me?

The service is delivered by the following team members:

  • Consultant for General and Medicine for the elderly
  • Respiratory Consultants
  • Advanced Nurse Practitioners
  • Nursing Staff
  • Allied Health Professionals
  • Administrative Staff

What does the Hospital at Home team do?

The team will provide treatment and support whilst monitoring your condition in the comfort of your own home. One of the Clinicians will review your case regularly and agree how often contact will be made and feedback to the rest of the team.

You may also be referred to hospital for investigations if required.

You will remain under the care of the Hospital at Home team until you are well enough to be discharged from the service.

What if I become unwell?

If you begin to feel unwell outside of operating hours (after 6:00pm and before 8:00am), please call NHS 24 on 111 and they will give you further advice

If you require immediate attention and feel that it is a life-threatening situation then dial 999
