Clinical transfusion policy

The aim of this policy is to ensure that the right blood is given to the right patient at the right time,
every time. 
This policy is for all staff involved in requesting, prescribing, taking samples for, supplying, transporting and
administering blood and blood components. It takes into account national guidelines and directives; the Blood
Safety and Quality Regulations 2005 (BSQR), the annual Serious Hazards of Transfusion reports (SHOT) and the British Committee for Standards in Haematology (BSH) (2016) Transfusion for Fetuses, Neonates and Older Children.

The policy provides information for all staff involved in the transfusion process. It is intended to reduce and
ultimately eliminate the risk of transfusion errors. Where there may be existing differences in practice across
NHS Greater Glasgow and Clyde (NHS GG&C) this will be highlighted and information for individual areas will
be provided.
