Gentamicin Prescribing
Children and Neonates
Refer to RACH / AMH Guidelines [intranet links only]
Adults
Refer to the full gentamicin guideline for adults (Hartford regime) for further details on prescribing, administering and monitoring.
Use the adult calculator
It is good practice to print a copy of the result if possible. The printed gentamicin dose calculation should be filed with the patient's gentamicin prescription and administration record.
Please prescribe gentamicin on the (red) gentamicin prescription, administration and monitoring chart - available on wards or via your ward pharmacist. If your ward area use HEPMA, gentamicin must also be prescribed as a 'PRN' selecting 'Gentamicin (ADULTS) Infusion AS PER PAPER CHART' with the dose entered as ‘1 dose’ and the frequency field left blank.
For synergistic use in patients with endocarditis refer to Synergistic Gentamicin for Endocarditis in Adults Guideline.
